This rule applies only to roots that have a second vowel. The "second one-voweled part" starts after the first vowel, and extends up to the second vowel, inclusive.
Example. Adding /san after the root aT अट् we get aTiSa अटिष (with /iT), a derivative root for "want to wander". (This aTiSa अटिष is a root because sanAdyantAdh... says so.)
The first part of aTiSa अटिष is a अ, and the second part is Ti टि.
By sanyaGoH 7>, aTiSa अटिष must reduplicate.
By this rule, when aTiSa अटिष reduplicates, two Ti टि replace the Ti टि. So we get —
aTiTiSa अटिटिष
as in —
aT अट् + /san + /laT /tip → **aTiTiSati अटिटिषति "wants to go walkabout"
