This change is called /Natvam ("Na ण-ness").
vistR- विस्तृॱ + /kta
→ vistIr- विस्तीर् + na न
→ **vistIrNa- विस्तीर्णॱ "extended"
saGkIr सङ्कीर् + na- नॱ → **saGkIrNa- सङ्कीर्णॱ "narrow"
**pitR- पितृॱ + /Am
→ pitRR- पितॄॱ + /nAm
→ **pitRRNAm पितॄणाम् "of fathers"
There is a very important difference between the /Natvam rules and the /Satvam rules. The /Satvam will only change into S ष् that s स् that is at the start of an affix (or of a Sa-root); /Natvam however will change n न् everywhere —
**brahman- ब्रह्मन् + /Gas → !**brahmanas ब्रह्मनस्

Why does your translation mention R ऋ RR ॠ? The ra;Sa रॱष in the sUtra only means r र् S ष्!
There is a /vArttika, RvarNAccetivaktavyam ऋवर्णाच्चेतिवक्तव्यम् "after the /savarNa of R ऋ too". Those /savarNa of R ऋ, according to commentarists, are R ऋ, RR ॠ, and L ऌ.
